Membership Benefits, Details, and Exclusions
- No yearly maximums, deductibles, or pre-authorization requirements
- Membership plans are valid for 12 months from the effective date and are non-transferable.
- Membership dues are due in full upon enrollment and are non-refundable. Plan fees are reviewed annually and are subject to change.
- Membership plans cannot be used in conjunction with any dental insurance plan.
- Services related to accidents, legal matters, medical or homeowner’s insurance claims, or workers’ compensation are excluded from this plan.
- Services rendered by a dental or medical specialist that is not directly employed by Adorable Smiles Pediatric Dentistry are not covered by the Membership Plan.
